Rita Torres
Rita Torres is the CEO and founder of Health Care Partners Foundation, a nonprofit organization that serves underserved and vulnerable populations in Colorado and New Mexico. With over 25 years of experience in healthcare administration and management, Rita specializes in program development, strategic planning, and medical management systems. She is a Certified Correctional Health Professional and oversees grant programs and medical and mental health services in rural detention centers, urgent care and primary care clinics. She was the project manager of a HRSA grant aimed at reducing recidivism and has successfully developed and implemented electronic medical records and telemedicine systems in rural detention facilities. Rita is a sought-after speaker, having been featured at multiple conferences, including the Legal and Mental Health Conference in Lyon, France and the 3rd Annual Correctional World Health Care Forum in Baltimore. She has been recognized for her contributions to healthcare through multiple awards, including Businesswoman of the Year by the American Business Women's Association and the local YWCA "Tribute to Women" award for her community-based healthcare programs for the underinsured and uninsured.
Medical Diretor
Tim Moser, MD
Dr. Moser is a highly experienced and versatile physician with a diverse background in a range of medical roles. They have served as a surgical intern, Chief Medical Officer of an Army Medical Clinic located on a chemical weapons site, Emergency Room physician in rural Colorado, full-time clinic faculty at a busy Kaiser clinic, performed disability evaluations for Social Security Disability applicants, worked as a solo practitioner in an innovative home-based primary care practice, served as Medical Director at county jails, and as Medical Director of a home hospice agency. Dr. Moser is well known for their exceptional organizational skills and punctuality, with a perfect attendance record in their ten years at Kaiser Permanente. Their strong personal skills and sense of humor have made them a valuable team player, helping to create a positive and supportive work environment for their colleagues.
Operations Manager
Timothy Romero
With over 40 years of experience in Emergency Services, Mr. Romero is a seasoned professional with a wealth of knowledge and hands-on experience in law enforcement, firefighting, and emergency medical services. He has been on the front lines of major incidents such as the Stapleton plane crash and the Columbine shooting, and has coordinated a variety of medical events. In addition to his practical experience, Mr. Romero is committed to training and education, serving as the owner of an American Heart Association training center and a Colorado EMS training site. He is dedicated to promoting safety in the medical field, as evidenced by his book "EMS Street Survival: Surviving the City" and his focus on safety in his training programs.
Addiction Medicine Provider
Jennifer, NP-C
Jennifer is a highly experienced nurse practitioner with over 20 years in the field and 7 years of experience as a nurse practitioner. Her diverse background encompasses trauma ER, critical care, health risk assessments, family medicine, utilization review, mental health, leadership, management, research, and training. She is a Board-Certified Nurse Practitioner and has expertise in providing primary care, mental health care, substance use disorder treatment, and dual assessments to inmates in Colorado jails. With a passion for improving healthcare, Jennifer has experience leading teams and creating policies, and she currently practices via telehealth in 5 states and provides consulting services and research services to healthcare facilities and providers. She is also a trained MAT provider and has critical care and emergency medicine certifications.
Mental Health Provider
Christina, LPC
Christina is a driven Case Manager with over 10 years of experience in providing healthcare management services to members with government-funded/managed care insurance. She has a strong educational background and holds a license, demonstrating her expertise in the field. Christina excels at multi-tasking, communicating, assisting members with conflict resolution, and managing healthcare and well-being needs. Through the use of care management tools and comprehensive evaluations, she identifies and addresses high-risk factors and service needs, coordinates and implements assigned care plans, and assists with medical and behavioral health discharge planning. Christina is experienced in negotiating to secure necessary options and services for members, providing coaching and support to empower them, and performing case management in compliance with regulatory guidelines and company policies. She also has a proven track record of training new employees on workflow processes and integrated computer systems.

Aimee FNP-BC
Aimee Edmund is a highly skilled and experienced nurse practitioner with a wealth of knowledge in the healthcare industry. With over 30 years of experience, Aimee has developed a reputation for providing high-quality care to her patients. Aimee received her Master's Degree in Nursing from Thomas Jefferson University in Philadelphia, PA in 2008, and has since gone on to earn a post-master's certificate in Family Medicine.

Throughout her career, Aimee has gained experience in various areas of healthcare, including geriatric care in both acute and long-term settings, oncology, family medicine, neurology, and mental health. Aimee is a Board-Certified Nurse Practitioner and has extensive experience providing primary care, mental health care, and dual assessments to inmates in Colorado jails.

Currently, Aimee owns and operates her own private practice, where she specializes in providing psychiatric evaluations, medication management, and family medicine. She also offers telehealth medicine to patients throughout Colorado, making healthcare accessible to people in remote areas. Aimee's passion for healthcare and her commitment to her patients make her a valuable asset to any healthcare team.
Sheriff, Las Animas County, CO
Derek Navarette
Sheriff Navarette is a seasoned law enforcement professional with 25 years of experience, including 23 years with the Las Animas County Sheriff's Office. He was elected Sheriff of Las Animas County in Trinidad, Colorado in 2018, and has since been dedicated to serving his community and improving the operations of his department.
One of Sheriff Navarette's most notable accomplishments is his involvement in the development of the Jail-to-Community Transition virtual telehealth program, which was funded through a HRSA federal grant and awarded to the Health Care Partners Foundation in 2017. Under his leadership, his facility became one of the first jails in Colorado to utilize telemedicine and telecounseling services in 2011, and was later selected to participate in a pilot program through the Colorado Office of Behavioral Health JBBS to demonstrate medication consistency through HIE integration for jail clients.
Sheriff Navarette is committed to providing quality healthcare services to those in his custody, and is constantly exploring new and innovative ways to improve the continuity of care for those transitioning from jail back into the community. His expertise and dedication to the field of corrections make him a valuable asset to his department and the communities he serves.
